If any tests have been ordered or performed, please bring with you any reports, referrals, tests, or any other items that may be beneficial for the physician to review.

Please bring your insurance cards and picture identification. We will need to make copies of both. Please bring a referral/authorization number if your insurance company may require one. Please bring and up to date medication list. Including vitamins, non-prescription drugs, and herbal supplements that you may be taking or have taken recently.
